To have the same person award the same person a medal (aka, the Obama Awards Obama a Medal meme), use character tags with emphasis and/or a character LoRA. In my experience regional prompting works the best (see below).
The from front LoRA works a bit better with the same person, probably because of the looking at mirror tag.
